Below is a link to the working online demo version of the COPCS.  This demo provides the experience of having the COPCS administered and provides a scored output report based upon the answers provided. 

Note: This demo version is not connected to a database so answers and the output report will not be stored or retained once completed.

Note: The COPCS does not provide a diagnosis; rather a screening for each of the 10 classifications as well as the estimated number of conditions co-existing within a given individual.
